The extra waypoint is for a missed approach. If you have to go around, you would be directed to that waypoint in real life
But when I land I have LNAV on past that point so what would I do then, also the ATC say what direction to head after executing a missed approach
A good thing to avoid this since that is an ILS cone, is to tune the runway to NAV1 then activate approach mode. That way you won’t be vectored to the missed approach waypoint
Hand fly probably. You would have A/P disengaged by then. Or you would switch from GPS to NAV1 in the avionics page. Or APPR.
I hand fly my landings but I don’t start hand flying until much closer to the airport. I turn off AP at around 3nm.
I never really use APPR, just preference.
Well even if you tune the the runway to NAV1, the localizer and glide slope will give a visual cue to land perfectly on the runway if you want to hand fly or adjust your autopilot accordingly

Is the missed approach fix not disappearing when you add your approach?
The missed approach dot is added with the approach.
Another issue I have is that some arrival tracks have the airport as a waypoint. This makes it so that approach tracks can’t be used unless removing the airport, but why are they added in the first place?
All of the SIDs and STARs are derived from real world procedures. At those airports Where the star ends at it, they most like get manual vectors as the approach the airport IRL

Sorry one more question, say that I am using a plane without APPR what should I do in that situation where it has a missed approach waypoint.
You can just follow the approach up until the turn for the MOF. Right there, you can disengage autopilot and land visually or on the ILS, etc.
