An Air Moldova Airbus A319-100, registration ER-AXM performing flight 9U-891 from Chisinau (Moldova) to Rome Fiumicino (Italy), was on approach to Fiumicino’s runway 16L when the crew initiated a missed approach at about 9.5nm before the runway threshold, positioned for another approach to runway 34L but went around from below 700 feet AGL. The crew subsequently decided to divert to Bologna (Italy) and were climbing through FL160 when the crew declared a fuel emergency, aborted the climb and decided to return to Fiumicino Airport. The aircraft however needed to go around again from about 1000 feet AGL while on final approach to runway 16L, performed a visual circuit and at 15:08L (14:08Z) landed safely on runway 16R about 48 minutes after the first missed approach.

Final Rpt. On Dec 1st 2017 Italy’s ANSV reported after collecting evidence the occurrence was rated a serious incident, an investigation has been opened. The ANSV reported the aircraft went around twice on approach to Fiumicino Airport due to bad weather, then set course to divert to Bologna but requested to return to Rome where another go around was needed. The crew performed a visual circuit and landed.

The occurrence aircraft remained on the ground for about 3.5 hours, then departed for the return flight.END

2). Incident: American E190 enroute on Nov 24th 2017, autopilot and rnav failure

An American Airlines Embraer ERJ-190, registration N952UW performing flight AA-2093 from Austin,TX to Philadelphia,PA (USA), was enroute at FL370 about 80nm southsouthwest of Nashville,TN (USA) when the autopilot and the RNAV equipment failed rendering the aircraft unable to maintain reduced vertical separation minima (RVSM). The aircraft descended out of RVSM airspace to FL270, switched to conventional navigation using VOR to VOR navigation and continued to Philadelphia for a safe landing about 100 minutes later.

The occurrence aircraft returned to service about 17 hours after landing.

    An Air Canada Airbus A320-200, registration C-FGYS performing flight AC-146 from Calgary,AB to Toronto,ON (Canada) with 117 passengers and 5 crew, was climbing out of Calgary when the crew reported a right hand engine (CFM56) failure, stopped the climb at 6000 feet, shut the engine down and returned to Calgary for a safe landing about 25 minutes after departure.

The Canadian TSB reported the crew received indication the right hand engine’s thrust reverser failed to lock. The crew worked the related ECAM procedures, secured the engine via a precautionary engine shut down due to a mild airframe buffeting. The crew declared PAN and performed an uneventful overweight landing. The aircraft was met by emergency services and taxied to the gate.END

Accident: Canada Rouge A319 at Toronto on Nov 19th 2017, laser strike injures first officer

An Air Canada Rouge Airbus A319-100, registration C-GJVY performing flight RV-1949 from Panama City (Panama) to Toronto,ON (Canada) with 113 passengers and 5 crew, was on approach to Toronto’s runway 24R about to intercept the localizer when the aircraft was hit by a green laser from the aircraft’s 12 o’clock position. Subsequently, while on a 4.4nm final approach the aircraft was struck by a green laser a second time hitting the first officer directly into the eye causing a minor eye injury. The captain reported the laser strike and continued for a safe landing.

The Canadian TSB reported several other aircraft also reported green laser activity. While on final approach the first officer, pilot monitoring, was looking at the window mounted electronic flight bag computer when the aircraft was struck by a green laser a second time and hit directly into the eye of the first officer, who experienced blurred vision and discomfort for 10-15 minutes after the event. The TSB rated the injury a minor injury. Police reports were filed Nov 19th and Nov 20th due to laser activity from about 4nm final.
