If an accident occurs on an airplane, is it considered a violation?
What do you mean by accident? For example, crashing the plane doesn’t give you a violation.
Only overspeeding and aerobatics (in the vicinity of an airport) count as violations from an aircraft control point of view.

If a plane crashes, will the crashed plane be removed from the server?
Yes if you “crash”, you get immediately disconnected from the server.
Otherwise the “Infinite Flight World” would be full of crashed planes right about now…

Removed from server only because the game ended when you crash, back to home screen, not because you get banned from that server
