I was looking at the 777 Quick Reference Guide. Are flaps 15 ALWAYS supposed to be used for takeoff on the 777-200LR?
Not always, usually it would 5 but depending on if you’re heavier or not it would be 15. I might be mistaken but I think anything above around 67% load would be 15 or maybe a little heavier than that

Given a long enough runway, Flaps 5 is nearly always the optimum setting - and is used far more often at considerably higher weights than on the 777-300ER given the shorter fuselage and therefore lower tailstrike risk of the 777-200LR; as shown below:
This seems to be the case in Simbrief as well.
Even on long-hauls with the 77W I’ve seen them often use flaps 5 for takeoff at SFO, more times than 15 iirc. I assume the 28s are long enough that they can afford to use much of the runway & rotate at a fairly high speed in exchange for better climb performance, which I assume is a main reason why flaps 5 are used so much.
